Community Nursing - Watford and Three Rivers

Watford and Three Rivers Community Nursing teams cover 3 geographical localities;

  • Rickmansworth and Oxhey

The service is provided every day of the year from 8pm to 10pm

The Community Nursing team is comprised of:

Team Leader

Senior Qualified Specialist Practitioner
Within each geographical area a team leader is responsible for a number of nursing teams who collaborate with an identified group of GP surgeries (cluster) to co-ordination the safe delivery and provision of service. Ensuring that all staff are fully competent and possess the appropriate clinical skills to deliver a high standard of care to our patients.

District Nursing Sister / Band 6 Community Nurse
Qualified Specialist Practitioner, who leads a team of nurses to provide the day to day care of patients. Performing specialist assessments for patients admitted to the service to plan care requirements based on needs identified.

Community Nurse
A registered nurse working within the team delivering day to day care and clinical interventions to patients.

Health Care Assistants
Unqualified staff that assist the nurses to address the care needs of patients.

Administrative Staff
They assist the clinicians by taking referral information, dealing directly with patient concerns and queries, relaying messages to the teams in a timely manner to ensure the smooth running of the service.

In addition we have Community Matrons – very experienced and highly educated nurses who work in collaboration with Community Nurses, General Practitioners and Intermediate Care teams. Their patients mainly have long term conditions and require extra support to manage their condition and prevent them attending hospital unnecessarily.

We also have a Tissue Viability Specialist Nurse who offers expert advice, education programmes and support to nurses to enable them to treat patients with particularly complex wounds.
